How it Works
Step 1:
Referal
Please speak with your doctor about a referral for home hospice care.
Your doctor may submit the referral via BRIGHT or through the Singapore Hospice Council (SHC) Common Referral Form.
Step 2:
Initial Assessment at Home
Once referred and accepted under our care, a primary nurse will contact you to arrange a home visit. During this visit, we assess the patient’s clinical condition, emotional well-being, rehabilitation needs, and caregiver support requirements.
Step 3:
Ongoing Care & Support
Our team provides regular home visits and coordinates additional services where needed. We support both the patient and family throughout the course of care.

Patients with advanced cancer and a prognosis of less than 1 year may be referred by their doctor for hospice home care. Eligibility is assessed upon referral and acceptance under our programme
